Your osteo: Once the GP identifies the issue as a musculoskeletal problem, the osteo steps in as the rehabilitation coach. They assess your movement, muscle strength, and flexibility to design a personalised recovery plan. This recovery plan often includes a combination of hands-on manual therapy, along with home-based exercises and self-help strategies.
Your GP: Considers your health from a medical perspective. They may prescribe medication if needed to manage pain or inflammation, the GP will address any underlying medical conditions that might be contributing to your problem.

Navigating Your Healthcare Journey: Understanding Medicare Chronic Condition Management Plans (GPCCMP) at YouMove Osteopathy.

At YouMove Osteopathy in Mount Eliza, we believe in a collaborative approach to your health and recovery. For many musculoskeletal conditions, your journey to better health is best supported when your healthcare providers work together seamlessly. Our role, in conjunction with your General Practitioner (GP), is to provide you with comprehensive osteopathic care under the new Medicare Chronic Condition Management Plans (GPCCMP).
Effective July 1, 2025, the Australian Government is introducing the new GP Chronic Condition Management Plan (GPCCMP) to simplify and enhance the way chronic conditions are managed through Medicare. This new, streamlined plan replaces the previous GP Management Plans (GPMP) and Team Care Arrangements (TCA), making it easier for GPs to coordinate your care and for you to access the necessary allied health services.
The GPCCMP is designed for patients with chronic or terminal medical conditions that have been (or are likely to be) present for at least six months. It supports a structured approach to your care, fostering strong communication between your GP and allied health professionals like our osteopaths.
Our osteopaths at YouMove Osteopathy are integral to your recovery team. Here’s how the updated process will facilitate your access to our expert care:
- GP Assessment and Plan Creation: Your GP will assess your condition and, if appropriate, develop a GP Chronic Condition Management Plan. This plan outlines your health goals, the services you need, and the professionals involved in your care.
- Simplified Referrals: Under the new GPCCMP, your GP will directly refer you to YouMove Osteopathy via a referral letter, much like they would refer you to a specialist. The previous requirement for specific forms is being removed, making the referral process more straightforward.
- Accessing Your Sessions: Once referred, you can book your appointment with YouMove Osteopathy. Your initial consultation will involve a thorough case history, comprehensive assessment, and the development of a personalised recovery plan tailored to your needs and goals.
- Collaborative Communication: Throughout your care, we maintain open communication with your GP. We will provide updates on your progress after your initial visit and at the conclusion of your treatment plan, ensuring continuity and consistency in your care journey.
Number of Sessions: The new GPCCMP continues to provide access to a maximum of five (5) individual allied health sessions per calendar year. These sessions can be allocated across various allied health services, including osteopathy, as determined by your GP.
- Plan Validity and Reviews: To ensure ongoing access to allied health services, your GPCCMP must have been prepared or reviewed by your GP within the previous 18 months. This encourages regular check-ins with your GP to manage your chronic condition effectively. Your GP can prepare a new plan every 12 months if needed, and reviews can occur every three months.
- MyMedicare Registration: If you are registered with MyMedicare, you will access your GPCCMP and review items through your registered practice. If you are not registered, you can still access these services through your usual GP.
- Transition Period: There is a two-year transition period for these changes. If you had a GP Management Plan (GPMP) or Team Care Arrangement (TCA) in place before July 1, 2025, you can continue to access services under those plans until July 1, 2027. After this date, a GPCCMP will be required for ongoing allied health access.
